New Maryland Nursing Home Guide announced.
MAP of Worcester County is a centralized service to provide information & assistance for the individuals of all income levels who need long-term services or want to plan for future needs. Information is also available for family & care-givers trying to support a parent, spouse,or adult child.
Staff from the Worcester County Health Department, the Department of Social Services, and the Commission on Aging are located together to make it easier to access information and assistance across a wide range of programs.
The MAP program is comprised of the following programs:Worcester County Health Department
AERS
Adult Evaluation and Review Services is a program designed to assist persons who are"At risk" of institutionalization. A comprehensive multi disciplinary assessment is completed and referrals are made to available community resources which would allow them to remain in the community and achieve the highest possible level of independence and health.
Behavioral Health Services
Provides mental health and substance abuse evaluation services with linkage to community resources as needed. The Licensed professional is able to assess individuals in their home or other community setting.
Hospital Outreach Initiative
Through this program, a Nurse identifies county residents who are admitted to the local hospital or long term care facility and may be at risk for long term institutionalization. For eligible individuals, a comprehensive evaluation of needs is performed to identify home and community-based programs available. Recommendations of community services are provided to assist individuals in their efforts to return to their own home or community.
Medical Assistance Personal Care Program
Personal Care helps individuals with Medical Assistance who have a chronic illness, medical condition, or disability to remain in their homes. Services provided assist eligible individuals with basic activities of daily living.
Worcester County Department of Social Services
Adult Protective Services
Program which investigates reports of neglect, self-neglect, abuse, or exploitation of adults who are unable to protect their own interests & are at risk of immediate harm to their own person or to others. Serves adults 18 or over who lack the physical or mental capacity to provide for their own daily needs.
Social Services to Adults
Case management program designed to assist elderly, disabled & vulnerable adults, age 18 years & older, obtain needed home & community based long-term care services, equipment & programs.
In Home Aide Services
Assists functionally disabled adults with activities of daily living in their home, such as chores, personal care, transportation & other basic tasks to maintain the individual safely & independently in the community.
Respite Program
Program which helps with some financial reimbursement to the family of developmentally or functionally disabled individuals for provision of short term care.
Project Home
Provides Certified Adult Residential Environment (CARE) homes where disabled persons 18 years of age and older are cared for & supervised in a family-like setting in the community. Care providers assist with personal hygiene & other activities of daily living.
Worcester County Commission on Aging
Senior Care Program
helps frail, health-impaired older persons select and locate the services that can help them continue to live in their homes, such as personal care, light housekeeping, meal preparation, shopping, & transportation.
Senior Loan Closet
Equipment on loan: Walkers, Canes, Shower Chairs, etc.
Senior Outreach/Information and Assistance
Outreach brings information, assistance & comfort to the homes of those seniors in need. The service includes friendly visitors & telephone reassurance calls to homebound elderly.
