Disabilities Case Management Services
Infants and Toddlers Program provides, in conjunction with the Board of Education: evaluation, assessment, and case management; coordinates multiple services to developmentally
delayed infants (birth through age three). Home or clinic visits offered.
Eligibility:
- Experiencing at least a 25% delay in any developmental area.
- Manifest unusual development or behavior
- Have a diagnosed physical or mental condition that has a high probability of resulting in developmental delay
Developmental Disabilities Services includes case management, support, and referral for day and residential services for children age three through adulthood. A developmental disability is defined as a severe chronic disability of an individual, such as mental retardation, attributable to a physical or mental impairment (other than the sole diagnosis of mental illness) acquired before age 22. Home and clinic visits offered. Support group meets monthly at Health Department sites.
Case Management Services includes referrals for housing, financial, and support networking for Health Department clients of all ages.
Down Syndrome Clinic provides consultation and specialty services to parents and providers working with individuals with Down Syndrome, all ages, at quarterly clinics, and the Holly
