Homeless Outreach Team (HOT)

Close up of two hands, one is helping the otherThe Homeless Outreach Team (HOT) helps people in Worcester County who are experiencing chronic homelessness. HOT brings together staff from healthcare, housing, public safety, social services, and community organizations. Our team meets people where they are, listens to their needs, and helps them take steps toward greater stability and permanent housing. HOT services are FREE.

Their Choice Matters

HOT is here to offer support, options, and resources. People experiencing homelessness have the right to make their own choices about the help they receive. This includes choosing whether to accept services, treatment, resources, shelter, or housing.

Our team respects each person’s decisions and works at their pace. If someone chooses not to accept a service, HOT can continue to build trust, stay connected when possible, and offer support when they are ready.

How Can HOT Help?

Our team can help with:

  • Exploring permanent housing options. Housing is limited, and finding permanent housing may take time.
  • Connecting to emergency shelter when available and appropriate.
  • Getting healthcare and mental health services.
  • Connecting to substance use treatment and recovery support.
  • Applying for benefits and financial assistance.
  • Finding transportation to appointments and services.
  • Connecting to employment and job resources.
  • Replacing important documents, such as an ID or birth certificate.
  • Connecting to other helpful community resources.

How Do I Get Started?

If you or someone you know is experiencing chronic homelessness and needs help, contact the Snow Hill Health Department at 410-632-1100 and ask to speak with Maggie Moore. Our team can talk with you about their needs, explain available resources, and help determine the next steps.
