Snow Hill, MD- The Worcester County Health Department is proud to announce that the Community Health Assessment (CHA) survey is officially open to anyone who lives, works, or plays in Worcester County.
Your health and well-being while in Worcester County is important. The goal of the survey is to gather the community’s opinions about the health of themselves and the community overall. The Worcester County Health Department and partnering agencies will use the results of this survey and other information to identify the most important health issues that will be addressed in the 2026 Community Health Improvement Plan (CHIP).
“Participating in the community health survey is a great way for you to have your voice heard, to share what matters most to you. Your feedback, as well as other community data, will inform this process and assist in formulating our improvement plan,” said Rebecca Jones, Health Officer.
Everyone will have the opportunity to complete the survey through the end of September. The survey will take roughly 10 to 15 minutes to complete and can be accessed in a variety of ways such as:
- social media platforms, including Facebook, Instagram and X,
- the Worcester County Health Department website, worcesterhealth.org
- print materials, including flyers and postcards with the QR codes that link to the survey electronically
- hard copies of the survey can be found at any of the Worcester County Health Department locations, or upon request.
The survey is completely voluntary, anonymous, and all information provided will be kept confidential.
We ask community members and agencies to help share information about the survey so that we can reach as many people as possible. Everyone’s voice matters.
